PRE-SCHOOL/KINDERGARTEN REGISTRATION

 

Registration and screening for the 2010-2011   Pre-school & Kindergarten class will be held at Laurel Springs Elementary School, Grand Avenue, Laurel Springs, NJ, 08021, on Wednesday, March 3, 2010 between 9:00 a.m. and 11:00 a.m. and between 1:00 p.m. and 3:00 p.m. in the main office.  The registration procedure is outlined below.  Please call us at 783-1086 to set up a screening time.

 

ALL FORMS SHOULD BE PICKED UP AND FILLED OUT, IN ADVANCE.  IMMUNIZATIONS RECORDS ARE REQUIRED – PLEASE BRING THEM WITH YOU.

 

  1. Birth Certificate (Original):  To prove that your child will be five (5) years of age on or before October 1, 2010 for kindergarten and four (4) years of age on or before October 1, 2010 for preschool.

 

  1. Immunization Records

 

Note:  If your child was born in another country and is entering a NJ school for the first time or if your child is transferring into a NJ school from another country please see the school nurse.

 

  1. Physical Examination:  A physical examination must be performed by the family physician using a form supplied by the school.  The form is attached.

 

  1. Registration Form:  Attached

 

  1. Proof of custody, if applicable (legal affidavit required).

 

  1. Proof of residency (legal affidavit required):  Lease for apartment dwellers/tax bill for homeowners.

 

  1. Please complete NJ Smart Form (attached)

 

NOTE:  BY LAW, NO CHILD IS PERMITTED TO ENTER SCHOOL UNLESS IMMMUNIZATIONS ARE UP TO DATE.
